Caudal steroid injections are epidural injections performed through the sacral hiatus near the base of the spine. The caudal route can allow medication to spread through the lower epidural space toward irritated lumbar or sacral nerve roots.
They may be considered for selected people with radicular leg pain, spinal stenosis, disc-related inflammation, or persistent symptoms after lumbar surgery. They are not a routine treatment for nonspecific low back pain.
The goal is to reduce inflammation enough to improve function and participation in rehabilitation. The amount and duration of benefit vary, and a predetermined series should not be assumed.
Caudal steroid injections use fluoroscopic guidance, contrast, local anesthetic, and corticosteroid delivered through the sacral opening to treat selected lower-spine nerve inflammation.

Caudal steroid injections use one of several approaches to the epidural space. The route is chosen according to anatomy, prior surgery, symptom distribution, and the target level.
Corticosteroid is intended to reduce inflammation around sensitized nerve tissue. A local anesthetic may provide shorter-term numbing and can temporarily change symptoms before the steroid effect begins.
Fluoroscopy and contrast are used to confirm position and medication spread unless a documented contraindication changes the plan. Image guidance helps avoid relying on surface anatomy alone.
Candidacy is based on a clinical diagnosis of radicular or epidural inflammatory pain and an acceptable safety profile.

Disc material and the surrounding inflammatory response can irritate nearby nerves, producing radiating pain and neurologic symptoms.

Your clinician reviews where symptoms begin, whether pain travels, how long it has been present, what worsens or relieves it, and how it affects walking, work, sleep, and daily activity.
Strength, sensation, reflexes, movement, tenderness, gait, and nerve-tension signs help distinguish joint, disc, nerve, muscle, and other potential pain sources.
MRI, CT, or X-rays are reviewed when results are likely to clarify anatomy or change the plan. Prior procedure reports and treatment responses may also be important.
Blood thinners, diabetes, allergies, infection risk, pregnancy, implanted devices, and other medical conditions are discussed so benefits and risks can be considered safely.
The process is typically brief, but preparation and aftercare instructions vary. Follow the directions from your treating team.
The patient is positioned, monitoring is applied, and the skin over the lower sacrum is cleaned and numbed.
Fluoroscopy helps guide a needle through the sacral hiatus into the caudal epidural space.
A small amount of contrast confirms epidural placement and displays the pattern of spread toward the intended region.
Corticosteroid, often with local anesthetic or saline, is injected slowly while symptoms and resistance are monitored.
The needle is removed and the patient is observed for short-term numbness, weakness, reaction, or other concerns before discharge.

Local anesthetic and corticosteroid act on different timelines. A temporary flare does not automatically mean the procedure failed, and immediate numbness does not predict the full result.
Temporary soreness, pressure, numbness, or leg heaviness may occur. Arrange transportation and avoid unsafe activity while strength or sensation is altered.
Some people notice improvement within several days, while others need longer or do not benefit. Symptoms may fluctuate as the anesthetic wears off.
Return to activity according to instructions. When pain is calmer, a graded rehabilitation program may help improve strength, mobility, and tolerance.
Track leg pain, walking, sleep, medication use, and activity. A meaningful functional change is more informative than a brief change on the procedure day.
A caudal injection may create a temporary window for recovery. It does not reverse every structural change or guarantee avoidance of surgery.
Risks include bleeding, infection, headache, temporary pain increase, nerve injury, medication reaction, elevated blood sugar, fluid retention, flushing, and failure to improve. Rare serious neurologic complications are possible.
Call 911 for severe weakness, breathing difficulty, a major allergic reaction, or another life-threatening emergency.
Injection is most useful when it supports a defined functional and rehabilitation goal.
Mobility, nerve-tolerance work, strength, walking progression, and body mechanics may help preserve improvement and build resilience.
Nonopioid, anti-inflammatory, or nerve-pain medication may be continued or adjusted according to the diagnosis and response.
Transforaminal or interlaminar injection may provide a more focused or anatomically appropriate route for some patients.
Progressive neurologic loss, structural instability, or disabling symptoms despite care may prompt surgical or advanced pain-treatment review.
